Wiki 99214 Not paying

We have a provider that always got payment for a 99214 previously and now all of sudden insurance (UHC,UMR) are writing off the whole amount billed. This is for mental health diagnosis. This procedure had paid previously and with the same diagnosis. We are seeing an entire write off??? Has anyone esle had this problem, if so please expand.
