Wiki 99214 OR 99215? need assitance

I feel its a 99214 but then I feel its a 99215 because per e/m guidelines we are able to met or exceed


HPI 4 elements
ROS 5
PHM 1 reviewed medication
FH 1 (r/o breast cancer)
SH 1 (former smoker)


Exam = 6 bullets EPF

MDM= High
PPT 3 PTS (had bronchitis took meds and not feeling better)
DATA minimal 0
Risk - one chronic illness with severe exacerbation (Pneumonia)

Could someone review and provide input.
 
I would agree with the assessment of a 99214:

1. HPI with 4-HPI, 5-ROS, and PFSH would be a 99214
2. Exam with 6 components would be a 99214
3. MDM comes in at a 99214
