Can anyone give me examples of 99214 justifications during a skin exam.
 
An example could be:
A patient has a long history plaque psoriasis. They are being seen in follow up. They were prescribed triamcinolone at last visit. The plaque psoriasis has not responded well to application of triamcinolone. Patient will be started on Humira.

Rational for 99214:
Patient has a chronic disorder that is not well controlled and is requiring prescription medication.
