denied claim for medicare. 99223 was billed out 3/2/?? and a 99232 3/3/?? Why is this being denied?

maybe

the denial could be because of the number of days. they might need to see the admit and discharge hours on the claim proving that it was more than a 24 hour stay.
 
Was your physician the admitting provider? Were other physicians involved in the care of this patient? Do the hospital admit and discharge dates match yours?
 
When reviewing your medicare remittance advice check under the patients name for REM: there should be more codes to give you more specifics about the denial
