Hello,
I have a provider who billed a 99225 (subsequent observation care) for the below encounter. Do you believe this is enough to meet for a 99225? Please provide some sort of rationale with you answer if you have enough time.

Constitutional: NAD VS Temp: 97.9 HR: 70 RR:20 BP:125/75
Cardiovascular: RRR, no MRG’s No edema
Lungs: CTAB CRW Resp unlabored
1. CP- MPS am
2. HTN- CAD cont current meds
3. PVD/ R BKA cont current meds
4. PAF cont current meds
5. DM cont current meds
6. HLP cont current meds
7. CVA/TIA cont current meds
8. RA cont current meds
And then there were also some hand written lab values
