If our doctor admitted a patient at 9 pm on 2/28/11, and the patient decided to sign out AMA 45 minutes later, should I code the 9922_ series, or use a code from the 99234_series. I can't find any clear indication of how to determine this. Also, there is no actual discharge summary.
