When billing a 99396 and a 99213(documentation supports a separate E&M), I know the 25 modifier goes on the 99213. However, if there are other services such as 90471 and 90656 or 69210 on the claim, would you put a 25 on the 99396 as well? If you could please send any links you have that support your answer I would appreciate it very much!
