We submitted 99401 with Dx code Z68.43 (BMI adult) but Horizon NJ Health denied stating "Missing/incomplete/invalid principal diagnosis". How do I fix this?

You probably figured out the answer by now, but BMI codes can't be listed as primary dx codes. Per ICD-10 "BMI codes should only be assigned when there is an associated, reportable diagnosis (such as obesity)".
