Hi, is it okay to bill 99407 with Z71.6? Do I need to add modifier 25 when it's with an E/M code?
 
Yes, you can bill 99407 with Z71.6 as the diagnosis... that's the correct ICD-10 code to support it when the patient is not currently diagnosed with a tobacco related condition. If the patient has a condition related to tobacco use (e.g., COPD, nicotine dependence, cardiovascular disease), those codes may also be appropriate depending on the clinical scenario. When 99407 is billed with an E/M code, you do need to add modifier 25 to show that it was a separately identifiable service. Just make sure the documentation clearly supports both: a problem-orientated or preventative E/M and a distinct intensive cessation counseling session > 10 minutes.
