Hello,
Can 99417 (prolonged services) and 96374 / 96361 (infusion codes) be billed at the same time? I am getting denials stating the insurance does not pay for both (99417 and 96374 / 96361) in the same visit because they are "timed codes" and it is considered "double dipping" I billed it as 99215-25, 99417, 96374 and 96361. Also, should I use modifier 59 on the infusion codes?
 
They would need to be unrelated. The E&M and the drug admin would need to be separate anyway. If your documentation supports that, I would appeal. I would first double check and make sure it's separate.

Were the injection and infusion done during the same visit or were they done separately? Also were they done by someone else? If so, then add the 59
