99455 - Patient came in for a work

Patient came in for a work PE - he has DX of 250.02, 414.01, 401.9 and 296.80. In terms of ICD-9 coding for the visit, would you use V70.5 as primary and then all associated DX as secondary codes or only use the V70.5? thanks

Use the V70.5, use the associated dx as secondary only if they were managed, controlled, or treated at this encounter.
