Can someone please help me with understanding when it is appropriate to use this code? I mean is it whenever the doctor rates the patient? Or????
 
Yes you would use this code when a physician assigns a disability rating to a patient for either a work related or other medical disability. However, be aware that most insurance companies will not pay this code. If it's a Work Comp claim you need to check with your state Worker's Comp Commission, CT Worker's Comp requires CPT code 99214 for all ratings, so it's important to know what your particular state wants.

I've usually used this code when a lawyer requests a rating in those cases docs required payment up front. Always best to check with individual payers to find out what they cover.
