In a teaching facility, when a baby is born and discharged on the same day, and there is a H&P and discharge note on the same day, is it a requirement that the attending physician apply an attestation statement to both reports that he examined the patient for billing? My gut says yes but I cannot find any documentation to support what I think. Thanks.
