When billing a 99463 initial visit & discharge the same day is the physician required to make 2 separate notes? Do we need a H&P and Discharge note separate or can all of it be on one note? We have always made 2 notes - H&P and Discharge but our physicians are wondering if this is redundant and unnecessary? I can't find an answer anywhere on this...Can someone help me out please?
