Does anyone know if documentation needs to support all of the elements of this code in order to bill it which include: monitorings of vitals, respiratory/cardiac/lab and oxygen also heat maintenance/nutritional adjustments. Or can just one element documented support the 99477, for example, if a mother had gestiational diabetes and baby glucose was resulted 3 x on the initial day, but that was all? Thanks
