I have a provider who does CCM, he has several patients that he bills 99487 for the first 60 minutes and then 99489 for each additional 30 minutes. I am getting denials for CO-151 ( Payment adjusted because the payer deems the information submitted does not support this many services.) I didn't realize there was a max amount of times you can bill the 99489. Anyone else running into this. I have not been able to find any documentation limiting the number of units. I may just have to file and appeal with documentation. Just wanted to see what everyone has run into.
