Would anyone know whether or not a PCP can bill using these codes for a patient being released from the hospital and following up with us as their PCP? Anyone know the guidelines or criteria that has to be met for billing purposes?

Try this link: https://www.cms.gov and type in those 2 codes in their search bar (top-right of the website). Then scroll down to Transitional Care Management Services Text Only. I think that document might be helpful..
