Wiki 99495 when seen at another hospital

One of our docs has been seeing patients and entering charges for Transitional Care codes, but the pts were discharged from another local hospital. I cannot find any information on the guidelines if the 99495 or 99496 can be used (or not used) if the patient was seen in a different hospital than the one associated with the clinic the patient was seen at. All of the other criteria is met. Thank you for helping, Pam
