Has anyone noticed or seen that Medicare is paying for the 92136 26 RT/LT, but not paying on the 92136 TC? How can we fix this? It doesn't happen on all patients, just a few here and there.
 
Payment adjusted. Payer deems info submitted doesn't support # of services. Information furnished doesn't substantiate the need for service.

However this is on random Medicare members. It is not all Medicare members.
