I received a rejections from Aetna Better Health for professional inpatient stay visits, i.e. 99223, 99232. They originally denied stating "Modifier used is incorrect or modifier missing." I did originally bill it without one. I added AF and rebilled as corrected. They denied that one for the same reason. I then billed it with AM modifier, they also have denied that for the same reason. I have gone on their website, no help. I even called and the rep could not help me. Can anyone help me on this issue? I would truly appreciate any help. This insurance is the only one I seem to have an issue with. Thank you.
