Wiki ABN denials. Any advice?

I work at a clinic that specializes in TMD and our providers are dentist(orofacial pain specialists) who bill medically because we treat the joint and not the teeth. We take CT scans and bill 70486, but have ran into an issue with pateints that have Medicare. We know that Medicare will deny these claims being that a dentist is the ordering provider and the diagnosis is not an approved indicator for the scan. We have tried obtaining an ABN and submitting with modifer GY, and GX but this still denies as provider liability saying "provider type may not bill this service/procedure" leaving us unable to bill the patient. Its fine if the patient checks box 2 but if they check box 1 then we have to bill it. Is there a different way I can submit these that will allow us to bill the patient? Any advice would be appriciated.
