If a patient presents for a screening colonoscopy and ACTIVE disease process is found (polyp/cancer) is found should the personal history (polyp/cancer) Z code be reported as well??? Per ICD-10-CM coding guidelines, it would be inappropriate and 3M Encoder specifically states "when the patient still has the condition mentioned or is under treatment for the condition, a code from categories Z85-Z92 is not assigned."
Can anyone provide specific information with references?
