Wiki Activity Codes on follow up visits

Do you have to code activity codes on follow up visits? So say a patient was injured riding a bike at a recreation area. On the initial visit we code the occurrence and activity code but must we code it on follow up visits for that same injury?

No the guidelines will tell you that place and activity are used only for the initial encounters
