The acupuncturist at our clinic went to a seminar about billing acu to Medicare. They told him that a 1490S form needs to be submitted first to get the denial so it can be sent to secondary. Does this need to be filled out and sent in per date of service or per patient?

Per patient encounter. The instructions on the back of the form tell you exactly how to use it, although I've never used one personally.
