Ok so for dx "acute chest pain" would it be 338.19 (pain, acute) and 786.50 (pain,chest) or just 786.50 (pain,chest)? Thanks!

Short description: Acute pain NEC.

Since the pain is "chest pain", this is a more accurate description than the NEC pain. I would use 786.50.
 
I would use 786.50 for the chest pain and then 338.19 as an additional code but not the first listed.
