Can anyone tell me if lymph nodes from EACH relevant station need to be removed to report code 32674? Or can you report this add-on code if, say for example, lymph nodes from level 7, 10R and 4R only are dissected. I have only been coding cardiothoracic surgery for about a year now and could really use some help with this! Thanks to all
 
it doesn't specify which levels must be biopsied but it does state that it is a regional and mediastinal lymphadenectomy which is standard. Do you have the operative report handy?
