Wiki Adding the supervising physician in Advanced MD billing system

We have a problem with adding the supervising physician in Advanced MD billing system. This is a big issue, because the diabetic patients MUST have the supervising doctor's name attached on the claim. Does anybody know how to add it in Advanced MD? I would really appreciate your help. Thank you
