Members that had Breast Cancer frequently get oral adjuvant therapy (Arimidex, Tamoxifen) after the malignancy was surgically remove, and treatment (chemo or radiation) was completed. Will the breast cancer still be coded as active (C50.-) or Hx of neoplasm (Z85.3)? Thanks,
 
Depends on documentation. If the record was documented to indicate above, then you'd use hx of since the cancer at the site has been eradicated and this is prophylactic treatment. But the provider has to indicate both lack of cancer and intent of therapy accordingly.
