as long as your provider documents a visit that is over above and beyond the reason for the injection then it should not bundle, you need a 25 modifier on the E&M.
Many payers do however auto deny either the E&M or the injection, due to the significant number of audits that have revealed that there is no significantly different E&M service. The expectation is thtyou will appeal with the documentation to support the usage of both.
