Who's responsible for the H&P when it comes to admitting, attending or primary physician? I have physicians saying it's someone else's responsibility and nobody is doing anything to get it done. Who does it at your facility?
 
At our hospitals, we have hospitalists (with the exception of two primary care practices) that make the decision as to whether a patient is admitted or not and they dictate the H&P. The two primary care practices that we have that follow their own patients in the hospital also do the H&P if they admit the patient.
