I am sorry I writing under wrong thread (PEDS). My question is for Adolescent clinic. Patient comes for Health maintenance and requesting the provider to fill out the forms (Camp/School/etc.,)V68.89.Is it Ok to add E/m along with modifier 25.As per physicians it takes time.I know it is nothing to do with medical necessity .

if they can use E/M how would you determine the level of service 99212/99213
