I am new to the forum. I am reviewing a refund request from Aetna for a 2013 DOS. The patient had mastectomy and reconstruction a decade ago with gel implant. Patient now has some deformity to implants and requested new implants. Procedure performed and billed CPT 19731 LT, 19340 LT, 19371 RT, 19340 RT. Insurance approved payments of 19731 LT $867.15; 19340 LT $961.62; 19371 RT $216.78; 19340 RT $480.81. Now Aetna is denying the left breast stating this is a duplicate claim. I assume that is becasue of the Right Breast. Thoughts
