Please advise me and read provider assessment below and my thoughts on the diagnosis coding:
Assessment
- Symptoms are consistent with allergic reaction to mosquito bites.
1. Insect bite of right forearm, initial encounter
2. Urticaria

I am confused as to if the "S" code is applied primary or if the Allergic Urticaria is applied primary. I can't find the guidelines on insect bites with a reaction.
My thoughts:
1) S50.861A, L50.0, W57.XXXA
2) L50.0, S50.861A, W57.XXXA
