We billed out a 90772 (admin of inj) J3420(b12 drug) and 95117. The insurance company will pay for the 95117 and the J3420. But they will not pay for the 90772 because they state that it is not pd seperately. When I looked at these codes I thought that they all should be paid. Am I wrong? Any help is greatly appreciated!!!
