I have a patient that has UMR as primary and Medicare as secondary. We billed 95165 with 230 units, the UMR paid and applied a portion to the patients deductible. We sent to Medicare and they denied since they do not pay for more than 20 units at a time. Does someone have suggestion on how to bill Medicare so they will pay?
