Does anyone have the written guidelines stating what documentation is needed to be an appropriate "interpretation and Report" for allergy testing?

Currently the RN tests the patients, fills out the allergy testing worksheet. That goes to the MD who signs one of the following statements on the bottom:

"I have personally reviewed this allergy test and will review it and discuss further management based on this testing with the patient by phone or at their next scheduled appointment."

or

" I have personally reviewed this allergy test and agree with the above findings written by RN. Allergy testing is positive in several categories based on wheal response. Will discuss further management based on this testing with the patient at their next scheduled appointment."

Is this enough? Does a more detailed report need to be done? If that is the case can you provide me with the documentation to support that?
