Aloxi / diagnosis code

 I sure hope someone can help me here. Our cancer center administers aloxi as part of a chemotherapy protocol. In order for this medication to be covered by medicare, it has to be billed with E930.7, E933.1, or V07.8. Since this medication is given as a prophylactic treatment, I am thinking that I can use V07.8? Here is my question. Does the cancer center have to document (for EACH VISIT of chemotherapy) that the aloxi is given prophylactically for nausea, or is the orginal order/protocol sufficient for documentation. Many thanks in advance and I appreciate any and all advise!
