Wiki Amendments to medical records

If an amemdment and / or correction is being made to a medical record --- is this to be amended and / or corrected by the provider themselves or can they delegate this to another staff member?
Where can I find the information on this as well.

There are situations where someone other than the provider (like a scribe) may realize a correction is needed, but just like the original record, it should be signed by the provider. You cannot have a staff member correct a record without it being signed.
