Can anemia 280.0 be coded as pdx as a co-equal in this case?

Pat. s/p polpectomy & admitted to hospital 2nd GI Bleed & Anemia. Pat. was transfused on 3 differend days & LAB monitored. Colonoscopy done & bleed was 2nd to the polypectomy.

Which would be pdx or is this co-equal: 998.11 or 280.0?

I am interested in hearing from an expert as well, but would the dx be 285.9 since iron deficiency was not specified?

I guess by definiton secondary would indicate a manifestation so would 998.11 be primary then?
 
Dr. did write blood loss anemia secondary to polpectomy .. code 280.0. since patient got transfused 3x i thought i could use 280.0 as pdx?
