Our CRNAs are "medically supervised" by the operating surgeons at our hospital. We are using the modifier QZ with our ASA codes.

My questions are:
1. As our CRNAs are "not medically directed" by the anesthesiologist. Which modifier is
correct, AD or QZ?
2. Is the maximum number of base units assigned to any ASA code to be limited to 3 as
the CRNAs are medically supervised by the operating surgeon?
3. What requirements must be met by our operating surgeons to provide medical
supervision to the CRNAs?
