Anesthesia coding per hour and pay per chart range

I have reviewed some old post threads and am curious if the expectation is still to code 20 anesthesia cases per hour if coding from both the anesthesia record and operative report, and 30 cases per hour if coding from only the anesthesia record? Also, do those targets include charge entry? Or only coding?

What is the range to pay "per case coded"? I know bread & butter cases are obviously coded faster compared to more complex cases such as cardio, thoracic, and perhaps even pain clinic (pain management) cases.
