Wiki Anesthesia - CPT 01924 Modifier Rejection

The CPT 01924 - Q6, QS, AA, P3 was billed under POS 11 to a substitute physician for a Medicare insurance. We are getting a rejection from clearing house due to invalid modifier. Kindly advise whether all the modifiers are valid and suggest me the possible way to post the charges from the rejections.
