Hi. I wanted to know where others pull the diagnosis for anesthesia services? Pre-anethesia note and/or Surgeon note or somewhere else?

I always preferred the operative report as the diagnosis and procedures would be tied more closely and less likely to be questioned by the insurance carrier.
