Does anyone have a sheet with the diagnosis that support medical necessity for anesthesia that they will be willing to share? We bill for ACS, Professional and Anesthesia charges. We have a very old out dated one that needs to be updated. I would like one that has more than just the DX and DX code, that may have something like "HTN well controlled or non controlled" Or "DM Type 1, Type 2, well controlled, poorly controlled" or the guidelines for MI...

I am trying to make one up...but its a bit overwhelming....

I was not able to find any sheet online.

Any suggestions, recommendations would be great.
