My question is this...

When billing from the Relative Value Guide you have a procedure like 27096 that has 7 base units (that is what we pay providers) and they are billing with a modifier 50. Do you pay

This is how they billed it

27096
27096 50

So is payment

27096 7 units * conversion factor
27096 7 units * conversion factor * 150%

or do you combine both lines and pay

27096 7 units * conversion factor * 150%

I hope someone knows this because I think it is the last example
