I have searched and cannot locate any templates for Anesthesia record , pre-op eval, and a billing sheet . I do not have logins or memberships to AMA or ASA . I have a new anesthesia group that is using horrible copies . You cannot even read most of the information. They are also handwriting on this record... Not the most favorable or compliant situation.

Any help or direction would be most appreciated.____________________________________
