Our Anesthesiologist wants to start charging for supervision of the CRNA's at our Critical Access Hospital. I cannot find much detail on the rules/guidelines surrounding this - besides that the AD modifier must be applied to the Anesthesiologist's charge and that there must be more than 4 concurrent cases in order to charge for this. Everything seems to lead back to "Medical Direction" which isn't what he wants to do.

How do we bill for for this?
Do the CRNA and Anesthesiologist charge for the same amount of minutes?
What needs to be in the documentation?

Any knowledge sharing about Anesthesia Supervision is welcome!
 
Can you explain what your Attending is doing on these cases? AD, as you stated, is when an Attending exceeds 4 concurrent cases or used when the Attending is performing other services while directing concurrent procedures.

Reimbursement for the Attending using AD is only 3 base units based on your contracted reimbursement rates. Time is not a factor in AD-Attenending cases.
