Wiki anesthesia with ED charge

We are seeing denials with our ED charges when we use anesthesia codes. They are saying ED charges are inclusive to the anesthesia charges. We have two providers, the ED doc adminsters ketamine to child, and ortho sets fracture.
Can someone explain why this would happen? :confused:
