CMS Annual Wellness Requirements link In the Link for Initial Component #8 and Subsequent Component #7 - Establish a list of patient risk factors and conditions where primary, secondary, or tertiary interventions are recommended or underway - Is it correct to direct the providers in their documentation to list, status and current treatment of the chronic conditions as part of this particular component?
