What is the coverage policy for annual wellness visit(G0438) and 99385 range of codes. How many times in 12 months can the exam be billed. for ex If exam is billed in Aug 2012 and if Dr wants to do it in July 2013, even if 12 months are not complete, in this case can we bill the exam. Is it 12 months or calender year wise coverage
